Confirm whether the clinician, dental service, and pharmacy are in network; whether a prescription drug benefit applies; and whether prior steps or documentation are required. Record the representative, date, and reference number when available.

Confirm the cash price, coverage response, generic product information, dispensing charge, delivery terms, and pharmacist access. A pharmacy quote does not include dental treatment unless it says so explicitly.
Ask the clinical service

Keep eligibility separate
