Tooth pain alone does not establish a bacterial infection. Decay, a cracked tooth, inflamed pulp, gum disease, a localized abscess, and pain after treatment can produce overlapping symptoms. The clinician needs enough detail to identify the likely source and judge whether delay could allow swelling or infection to spread.

The American Dental Association recommends prioritizing definitive dental treatment for most pulpal and periapical pain and localized swelling when that care is available. Antibiotics are not a substitute for drainage, root canal treatment, extraction, or another procedure that addresses the source. Systemic involvement such as fever or malaise can change the plan and urgency.
